Atrium Health Transition Services Clinics are outpatient post-discharge clinics in the greater Charlotte area, focused on helping patients restore body and mind after hospitalization. Our Transition Clinic is a multidisciplinary ambulatory Internal Medicine service for high-risk patients, offering integrated visits with physicians, pharmacists and social workers.

Ideal candidate values team-based care, internal medicine for adults, and improving access to care.

Qualifications

• Experience in Internal Medicine or outpatient care preferred.
• Interest in multidisciplinary collaboration and team-based care.
• Ability to engage complex patients to reduce readmissions.

Responsibilities

• Support post-discharge follow-up visits and care coordination.
• Collaborate with physicians, pharmacists and social workers in a multidisciplinary clinic.
• Provide patient education and support to improve access to care.

Atrium Health Transition Services Clinics are outpatient post-discharge clinics, committed to helping our patients restore their body and mind post hospitalization. Our Transition Clinic is a multidisciplinary ambulatory Internal Medicine clinic for patients at high risk of complications and readmissions due to medical complexity or barriers to healthcare access. We offer integrated visits with provider, pharmacist, and social worker. During our 30 day follow up period, we identify and alleviate barriers to health focusing on patient engagement, access, and education. Care is delivered in office, virtually and/or with community paramedicine support based on patient’s needs.

Highlights about the position include:

• 100% Outpatient
• Join current team of 3 Physicians & 2 APPs
• Office hours Monday-Friday (Flex (8)am-5pm)
• Call rotation: Shared coverage rotated a week at a time; phone only
• No Nights/Weekends
• Work at Transition clinic locations in greater Charlotte and provide virtual care.

Ideal candidate:

• Interest in Internal Medicine – Seeing patients 18 years old and up
• Passion for providing quality patient care to complex medical patients with focus on multidisciplinary collaboration
• Experience preferred
