Job summary

Sigma Health Rehab is actively seeking a Full-time Occupational Therapist (OT) to work in a Skilled Nursing Facility. The OT will evaluate residents and develop treatment plans to help them perform daily activities independently.

Candidates must graduate from an accredited OT program, hold a NY license, and have experience in geriatric or SNF settings; new grads with relevant experience are encouraged to apply. We offer mentorship, CEU reimbursement, and a comprehensive benefits package.

Qualifications

• Graduate of an accredited Occupational Therapy Program.
• Current New York State OT License.
• Experience in a skilled nursing or geriatric care setting preferred; new graduates with relevant experience encouraged to apply.

Responsibilities

• Evaluate Residents & develop an OT treatment plan to meet individual needs.
• Choose activities to help residents carry out daily activities independently.
• Observe, assess, and document progress with detailed reports.
• Maintain resident confidentiality and handle information securely.
• Foster a cooperative relationship among the healthcare team.
