Qualifications

• Licensed Occupational Therapist (OTR/L) in NM.
• Minimum 1 year of clinical experience; home health preferred.
• Strong problem-solving, communication, and time management skills.
• Comfort working independently in patients’ homes.
• Experience with EMR systems and mobile charting tools.
• Passion for enabling independence and dignity through skilled therapy.

Responsibilities

• Perform in-home assessments to determine functional impairments and therapy needs.
• Develop and implement individualized plans of care that support safety, self-care, and mobility.
• Provide hands-on interventions focused on ADLs, home safety, and patient-specific goals.
• Educate patients and caregivers in the use of adaptive equipment and techniques.
• Collaborate with physical therapists, nurses, aides, and physicians for coordinated care.
• Document progress and modify care plans in response to clinical changes.
• Support safe transitions and long-term functional wellness.
