Job summary

Corus Home Health in Albuquerque seeks a compassionate Registered Nurse to provide skilled home-based care. You’ll enjoy fieldwork autonomy, a collaborative clinical team, and the chance to grow within a mission-driven organization.

Responsibilities include assessing patients, developing care plans, administering nursing procedures, documenting OASIS, educating families, and coordinating with physicians and the inter-disciplinary team.

Qualifications

Active RN license in New Mexico (state licensure).
Minimum 1 year of clinical nursing experience; home health preferred.
Proficiency in OASIS documentation and home health regulations.
Current CPR certification.
Strong clinical judgment and attention to detail.
Ability to work independently with reliable transportation.
Valid driver’s license and auto insurance.

Responsibilities

Perform initial and ongoing assessments to develop and update personalized care plans.
Deliver skilled nursing services and specialized procedures per physician orders.
Complete timely, accurate documentation including OASIS assessments.
Educate patients and families on safety, medications, and health conditions.
Communicate with physicians regarding changes in patient condition.
Supervise and support LPNs and HHAs, including conducting supervisory visits.
Coordinate care with the interdisciplinary team and participate in team conferences.
Ensure compliance with all clinical, regulatory, and documentation standards.
