Job summary

Park Nicollet Health Services is seeking a Home Care Registered Nurse to assess, care for, and educate patients and their families in their home or community setting.

The RN will coordinate medical, social, and financial resources to develop comprehensive care plans, support admissions and discharges, and ensure quality, patient-centered care with compassion and professionalism across Minnesota service areas.

Qualifications

• Work as an RN for at least two years.
• Current MN Registered Nursing License.
• Current BLS certification required and maintained.
• Ability to provide care in home or community setting.

Responsibilities

• Assess and evaluate patient care needs in home setting.
• Provide skilled nursing care and teaching to patients and families.
• Coordinate medical, social, and financial resources to develop a plan of care.
• Support admission, discharge, billing, referrals, bereavement and care delivery.
