Job summary

Park Nicollet Health Services is seeking a Home Care Registered Nurse to assess, care for, and educate patients and their families in their home or community setting.

The RN will coordinate medical, social, and financial resources to develop comprehensive care plans, support admissions and discharges, and ensure quality, patient-centered care with compassion and professionalism across Minnesota service areas.

Qualifications

  • Work as an RN for at least two years.
  • Current MN Registered Nursing License.
  • Current BLS certification required and maintained.
  • Ability to provide care in home or community setting.

Responsibilities

Park Nicollet is looking to hire a Registered Nurse to join our Home Care team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary

The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.

The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.

The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.

Territory

Central (ex: St. Louis Park, Edina, Minneapolis)

Work Schedule
  • Monday - Friday 8:00-4:30pm
  • Every 6-7th weekend rotation
  • 1-2 Holidays per year.
  • Occasional on/call requirements
Required Qualifications
  • Education, Experience or Equivalent Combination:
    • Work experience as a RN for at least two years
  • Licensure/ Registration/ Certification:
    • Current MN Registered Nursing License
    • Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable. Homecare only
  • Knowledge, Skills, and Abilities:
    • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
    • Proven proficiency in the operation of the following: Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical records.
Preferred Qualifications
  • Experience in home care and or hospice desirable.
  • Must be able to function independently with minimal supervision.
  • High level of sensitivity, assessment skills, technical-skills and communications skills.

Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
