Qualifications

• 5 years nursing experience in an acute care hospital setting.

Responsibilities

• Assesses patient needs and coordinates appropriate level of care and bed placement to ensure safe, timely admissions and transfers.
• Facilitates inbound and outbound transfers, ensuring efficient communication and seamless coordination between referring and receiving providers and facilities.
• Serves as a central point of contact for physicians, facilities, and internal teams to support direct admits, ED transfers, and specialty activations.
• Ensures adherence to clinical protocols, regulatory standards, and patient safety practices throughout the transfer and admission process.
• Maintains accurate documentation, track key metrics, and support process improvements to enhance patient flow, timeliness, and service performance.
