Job summary

CHRISTUS Health is seeking a Registered Nurse for the Transfer Center in San Antonio, TX. The RN will coordinate patient transfers into the CHRISTUS Health System, gather clinical and demographic information, and communicate with referring and receiving physicians across facilities.

Responsibilities include providing outstanding customer service, prioritizing transfer needs, and working with ED staff, house supervisors, and administrators to ensure timely, compliant patient movement.

Qualifications

• Associate's Degree in Nursing required.
• RN license in state of employment or compact.
• Basic Life Support (BLS) certification.

Responsibilities

• Exhibits excellent customer service skills, including telephone and personal contact with all CHRISTUS Transfer Center groups.
• Maintains a high level of professionalism at all times.
• Maintains a patient-centered mentality at all times.
• Anticipates and prioritizes critical transfer issues.
• Escalates concerns/barriers that require administrative judgment per hospital policies.

• Accepts new assignments and performs additional duties professionally.
• Collaborates with team members using protocols and established resources for efficient transfers.
• Maintains timely knowledge of all current and future transfer requests.
• Collaborates with ED, House Supervisors, Administrators, and physicians for timely transfers.
• Works with House Supervisors to understand daily bed capacity and patient flow.
• Remains proficient in EMTALA.
• Organizes and prioritizes multiple complex tasks.
• Solves problems and makes quick decisions.
• Maintains proficiency in transfer center software and computer systems.
• Responds to callers with aid of computer systems and databases.
• Keeps knowledge of hospital medical staff and services up to date.
• Communicates with House Supervisors to access bed availability each shift.
• Updates service availability and contact information regularly.
• Keeps informed of physician/service changes by reading emails and attending meetings.
