Job summary

PruittHealth is hiring a Physical Therapist for Home Health in Florida. This full-time role offers evaluative and therapeutic services in patients' homes, with a new pay-per-point model tied to effort and time spent per visit.

The therapist will collaborate with physicians and families to plan and adjust care, maintain documentation, and ensure regulatory compliance across home health settings. Qualifications include a valid state license and graduation from an accredited Physical Therapy

Qualifications

• Must have a valid professional license in State of practice.
• Graduate of an accredited Physical Therapy Program.
• If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Evaluate the patient’s level of function and skilled therapy needs.
• Treat the patient to relieve pain, develop or restore function, and obtain maximum outcomes.
• Advise and consult the family and interdisciplinary team members to promote optimal outcomes.
• Assess, monitor, document and report progress using required documents and timelines.
• Maintain continuity of care by collaborating with the team and physician; referrals as needed.
• Demonstrate home care knowledge and competence for patient care skills.
• Conduct field visits to start of care and to monitor outcomes and care improvement initiatives.
• Utilize teaching materials in patient/family education.
• Complete documentation timely and accurately at the point of care.
• Meet agency productivity standards and participate in required trainings and meetings.
• Follow safety regulations and report occupational hazards as necessary.
• Maintain current license, certification, and clinical records.
• Prioritize care, show initiative, and adapt to workload changes.
• Report any violations to supervisor and resolve issues.
• Be self-directed, manage time effectively, and meet outcomes and deadlines.
