Qualifications

• Must have a valid professional license in the state of practice.
• Graduate of an accredited Physical Therapy Program.
• If foreign-trained, the program must be deemed substantially equivalent to a U.S. entry level education by a credential evaluation organization approved by the APTA.

Responsibilities

• Evaluate the patient’s function and skilled therapy needs.
• Treat to relieve pain, develop or restore function, and maximize outcomes.
• Advise and consult family and interdisciplinary team members to promote outcomes.
• Document progress using required documents and timelines.
• Collaborate with the care team to manage the care plan and referrals as needed.
• Maintain license, certifications, and clinical records as required.
