Qualifications

• Must hold a valid professional license in state of practice.
• Graduate of ASHA accredited program; foreign-trained must be substantially equivalent.

Responsibilities

• Complete evaluations, assessments, and treatments per care plan and timelines.
• Collaborate with interdisciplinary team to develop and manage the plan of care.
• Demonstrate home health knowledge and competency in required patient-care skills.
• Identify priorities in patient care and act with initiative and judgment.
• Assist with training and orientation of others.
• Utilize teaching materials for patient/family education
• Report suspected violations to supervisor until resolved.
• Comply with corporate compliance program
• Meet agency productivity standards and manage time/resources efficiently
• Participate in surveys, in-services, case conferences, staff meetings, and related programs
• Follow safety regulations and report occupational hazards
• Maintain current license, certification, and records as required
