Job summary

Devoted Health is seeking a TOC Supervisor to lead a team of Coordinators who help patients transition from hospital to home or post-acute care. You will oversee operations, coordinate care connections, and ensure resources are in place to prevent readmissions.

This role requires at least 2 years of supervisory experience in a virtual setting, MA certification preferred, and strong organizational skills. You will report to the Senior Manager and work in Kentucky.

Qualifications

• MA Certification (desired); active or expired is acceptable.
• 2 years of direct lead or supervisory experience in a virtual setting.

Responsibilities

• Lead a team of TOC Coordinators to support patient transitions.
• Coordinate with leadership to develop the TOC program and workflows.
• Recruit, hire, and onboard new TOC Coordinators.
• Monitor performance against KPIs and provide coaching.
• Participate in team rounds, huddles, and cross-functional meetings.
• Oversee scheduling and attendance of the TOC team.
• Drive process improvements and develop job aids.
