Registered Nurse Qualifications

Active RN license in Tennessee (TN Registered Nurse license required)
Minimum 2 years of recent experience as a Home Health RN
Proficiency with OASIS and HCHB (Homecare Homebase) documentation
Current CPR certification
Valid driver’s license, vehicle insurance, and reliable transportation
Fully COVID-19 vaccinated at time of application

Home Health RN Responsibilities

Deliver skilled nursing care to patients in their homes, following individualized care plans
Perform comprehensive patient assessments and document findings using OASIS and HCHB
Collaborate with interdisciplinary healthcare teams for holistic patient care
Educate patients and families on disease management, medications, and self-care
Monitor patient progress and update care plans as needed
Ensure accurate and timely clinical documentation in compliance with home health regulations
