Job summary

Tri-State LifeCare seeks a TLC Caregiver to provide in-home, non-medical support under the Hospice of Huntington TLC program. You will assist with daily living activities, offer companionship, help with light housekeeping, meal planning, errands, and transportation in clients' homes.

The role emphasizes safety, dignity, and respectful communication, with travel to client homes across WV counties and nearby OH areas.

Qualifications

• High School graduate, preferred.
• Proof of current nursing assistant certification, if applicable.
• Minimum one (1) year experience, preferred.
• Valid driver's license and automobile insurance, and reliable transportation with clean and smokefree interior.

Responsibilities

• Travel to multiple patient homes as scheduled during shift.
• Communicate schedule changes and care concerns to supervisor.
• Document in company system as required.
• Provide companionship and engage in social activities.
• Assist with light housekeeping and meal planning/preparation.
• Provide bathing/showering assistance and personal hygiene support.
• Run errands and accompany client to activities and appointments.
• Provide medication reminders and supervise home maintenance.

Job Title: TLC Caregiver
Reports to: Hospice Aide and Caregiving Coordinator
FLSA Status: Non-exempt

Position Scope: The Tri-State LifeCare Caregiving (TLC) program is a non-hospice program of Hospice of Huntington. The Tri-State LifeCare Caregiving program is a service line program of Tri-State LifeCare. TriState LifeCare Caregivers (TLC Caregiver) performs in-home services that are all non-medical services to individuals who require assistance with daily living activities. The following is a list of services that TLC Caregivers may routinely perform; however, it is not exclusive of other reasonable requests.

Essential Functions

• Travel to multiple patient homes as scheduled during shift. Travel is planned as efficiently and time conserving as possible with additional travel compensation but could include homes in WV counties Cabell, Lincoln, Wayne, and Mason, and/or OH counties Lawrence, Meigs, and Gallia.
• Responsible for communicating all schedule changes, concerns, and any other care related issues directly to their supervisor.
• Responsible for documenting in accordance with the company documentation system set forth.
• Caregiver is expected to interact with patient and in social settings with high integrity, politeness, respect and excellent communication.
• Caregiver is expected to be prepared to act in emergency situations as trained.
• Caregiver is expected to communicate with supervisor regarding patient needs and recommendations for care plan change.

Education and Work Experience

Work Environment and/or Physical Demands

The TLC Caregiver walks, sits, stands, bends, pushes and pulls, stoops, drives and lifts intermittently throughout the day. The TLC Caregiver must have sufficient strength to perform these duties.

Employment is contingent upon successful completion of a background check, drug screening, and possession of a valid driver's license.
