Job summary

Walden Family Services is seeking a THP Plus Case Manager in Los Angeles, CA. This non-exempt, full-time role coordinates with former foster youth and partners with agencies to help youth achieve goals.

Qualifications

• Bachelor's degree in social work or closely related field is required.
• Minimum of 1 year prior experience with the target youth population.
• Must possess valid California Driver’s License and a working vehicle.
• Fingerprint and Child Abuse Index clearances from the Department of Justice.

Responsibilities

• Coordinate with the placing agency, youth, and significant others to develop needs/services plans.
• Provide at least weekly consultation and case supervision with supervisor.
• Maintain up-to-date client files, intake forms, and progress documentation.
• Facilitate referrals to services (medical, therapy, housing, job training) and track outcomes.

WALDEN FAMILY SERVICESJOB DESCRIPTIONPOSITION TITLE: THP Plus Case ManagerEMPLOYEE STATUS: NON-EXEMPTPRIMARY SUPERVISOR: THP Plus SupervisorDESCRIPTION OF POSITION:The case manager will be responsible for working with former foster youth. Responsible for coordinating with significant others, placing agency, and adjunct treatment members to enable the youth to attain his or her goals. The case manager will not be responsible for providing after hour assistance to clients or on-call services.ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:Be knowledgeable of transitional housing programs and resources for transitional aged former foster youth.Conduct a pre-placement appraisal with the placing agency and the youth. Explain the program expectations and complete necessary intake paperwork.Following acceptance into the program the case manager will obtain intake information and complete a needs and services plan.THP Plus social worker will complete initial an update the needs and services plan/ STLP and submit to the supervisor within 10 working days.Provide at least weekly consultation to the youth. Consultation may but is not limited to discussion of progress towards goals, teaching life skills, teaching parenting skills, public benefits advice, housing advocacy and tenancy training, computer training, transportation assistance, assist in contact with appropriate supportive adults/mentors.To provide assistance with managing the youths cash resources, personal property, and valuablesTo provide assistance with maintaining accurate records of the youth’s bank account, savings, and monthly budget.To provide and/or coordinate referrals for additional services, consultants and community resources as needed (i.e. medical, therapy, psychiatric, parenting classes, job training, )Document the youths weekly progress at home, school, and/or work, major incidents, and other significant, collateral contacts.Complete special incident reports and any other reports as required by the placing agency.Maintain contact with placement worker and inform of any significant events/incidentsConsult supervisor regarding incidents, significant issues, and possible terminations, and follow-up as directed.Meet with supervisor for a minimum of one hour weekly of individual supervision and case consultation.Ensure that client files are complete and up-to-date.Submit all reports and documentation to supervisor for final approval.Facilitate trainings, and support groups as directed by supervisor.At all times project a favorable image of Walden Family Services.Performs other duties as assigned or requested.PHYSICAL DEMANDS:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
