Job summary

Walden Family Services in Riverside, CA seeks a dedicated case manager to support former foster youth. You will coordinate with placing agencies, families, and adjunct treatment to help youth achieve their goals.

This role does not include after-hours or on-call duties. Responsibilities include pre-placement appraisals, intake and needs/services plans, weekly client consultations, progress documentation, and collaboration with supervisors.

Qualifications

• Bachelor's degree in social work or closely related field; at least 1 year prior experience working with the population.
• Must possess valid California Driver’s License and have a good driving record.
• Fingerprint and Child Abuse Index clearances from the Department of Justice.
• Must meet all requirements set by Community Care Licensing pertaining to hiring by a licensed facility.

Responsibilities

• Be knowledgeable of transitional housing programs and resources for transitional aged former foster youth.
• Conduct a pre-placement appraisal with the placing agency and the youth, explain program expectations and complete intake paperwork.
• Following acceptance into the program obtain intake information and complete a needs and services plan; update as needed and submit within 10 days.
• Provide at least weekly consultation to the youth addressing progress toward goals, life skills, parenting skills, benefits, housing and tenancy training, transportation, and mentor contacts.
• Maintain accurate records of the youth’s cash resources, bank accounts, savings, and monthly budget; coordinate referrals as needed.
• Document the youth’s weekly progress at home, school, and work, including incidents and collateral contacts; complete required reports.
• Meet with supervisor weekly for supervision and case consultation; ensure client files are complete and up-to-date; submit reports for final approval.
• Facilitate trainings and support groups as directed; maintain a favorable image of Walden Family Services.

DESCRIPTION OF POSITION:The case manager will be responsible for working with former foster youth. Responsible for coordinating with significant others, placing agency, and adjunct treatment members to enable the youth to attain his or her goals. The case manager will not be responsible for providing after hour assistance to clients or on-call services.ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:Be knowledgeable of transitional housing programs and resources for transitional aged former foster youth.Conduct a pre-placement appraisal with the placing agency and the youth. Explain the program expectations and complete necessary intake paperwork.Following acceptance into the program the case manager will obtain intake information and complete a needs and services plan.THP Plus social worker will complete initial an update the needs and services plan/ STLP and submit to the supervisor within 10 working days.Provide at least weekly consultation to the youth. Consultation may but is not limited to discussion of progress towards goals, teaching life skills, teaching parenting skills, public benefits advice, housing advocacy and tenancy training, computer training, transportation assistance, assist in contact with appropriate supportive adults/mentors.To provide assistance with managing the youths cash resources, personal property, and valuablesTo provide assistance with maintaining accurate records of the youth’s bank account, savings, and monthly budget.To provide and/or coordinate referrals for additional services, consultants and community resources as needed (i.e. medical, therapy, psychiatric, parenting classes, job training, )Document the youths weekly progress at home, school, and/or work, major incidents, and other significant, collateral contacts.Complete special incident reports and any other reports as required by the placing agency.Maintain contact with placement worker and inform of any significant events/incidentsConsult supervisor regarding incidents, significant issues, and possible terminations, and follow-up as directed.Meet with supervisor for a minimum of one hour weekly of individual supervision and case consultation.Ensure that client files are complete and up-to-date.Submit all reports and documentation to supervisor for final approval.Facilitate trainings, and support groups as directed by supervisor.At all times project a favorable image of Walden Family Services.Performs other duties as assigned or requested.QUALIFICATIONS REQUIREMENTS:Candidate must have a minimum of a bachelor’s degree in social work or a closely related field, as defined in the State Department of Social Services, Community Care Licensing regulations with at least 1 year prior experience working with the populationMust possess valid California Driver’s License and have a good driving record.Fingerprint and Child Abuse Index clearances from the Department of Justice.Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility.
