Travel Requirement: Will be required to travel in Franklin and Licking County

Responsibilities may include:

1. Providing scheduled patient visits.
2. Ensuring treatment plans are up to date for each client.
3. Completing comprehensive and succinct progress notes.
4. Communicating and following up regarding the client's status.
5. Collaborating with case management and psychiatric service providers.

Requirements:

• Bachelor's degree in Social Work or a related field with one year of therapy experience, or a Master's degree in Social Work or Professional Counseling.
• Social Work/Professional Counseling License or equivalent (LSW, LISW, LISW-S, LPC, LPCC, LPCC-S).
• Experience in therapy and counseling preferred.
• Experience in a home setting preferred.
• Valid driver's license, car insurance, and reliable transportation.
• Willingness to travel throughout the assigned service area.
