Job summary

Cynet Health is seeking a Physical Therapist to work in Home Health. The role covers an entire service area with Monday through Friday hours, 8 AM to 5 PM, and requires travel between patient homes. You will evaluate and treat adults in their homes, typically seeing 5–10 patients daily.

Travel can be up to 75 miles weekly, and all charting must be completed within 24 hours using our EMR system. Eligibility includes a current PT license, CPR/BLS, and at least one year of Home Health experience.

Description:This position is for a Physical Therapist in the Home Health specialty.The contractor will cover the entire designated service area.The assignment is Monday through Friday from 8 AM to 5 PM.Travel is required, and the estimated total mileage per week is approximately 500 miles.The average number of patients seen daily will be between 5 and 10.Each visit and related activity will contribute to weekly productivity points, with a target of 30 points per week.All documentation must be completed within 24 hours of the appointment.The role requires the use of specific EMR systems for patient charting.Candidates must have reliable transportation, a valid driver's license, and liability insurance.The position requires flexibility in accepting all assigned patients regardless of distance.There are no guaranteed hours; productivity points must be met weekly.Candidates must be willing to travel up to 75 miles.Local candidates are accepted.On-call rotations may be required depending on location and job specifics.This position is not suitable for first-time travelers.
