Job summary

Cynet Health is seeking an Occupational Therapist to join our long-term care team for a 13-week contract. You will screen patients, develop care plans, and provide rehabilitative treatments aligned with regulatory and clinical standards. Strong documentation and communication with the health team are essential.

The role requires 1+ year of OT experience, an active OT license, and ability to supervise OT assistants. Days shifts, 8 hours per shift, on-site in Oklahoma.

Qualifications

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders.
• Develop and follow care plans aligned with regulatory and clinical standards.

• Provide rehabilitative treatments and ADL retraining as part of the plan.
• Record progress, adjust treatments, and document in the electronic system.
• Participate in meetings and trainings and communicate patient progress to the team.
• Supervise and support OT assistants and rehabilitation aides.

Description:Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders.Develops appropriate plan of care following all regulatory and clinical practice standards.Utilizes standardized assessments and evidence-based practice to support clinical interventions.Provides rehabilitative and medically necessary treatment intervention to patients per requirements.Administers various procedures as part of the rehabilitation plan including ADL retraining and therapeutic activities.Assesses patient progress and adjusts treatments accordingly in the clinical record.Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluation.Provides consultation and counseling to patients, families, caregivers, and other service providers.Effectively communicates with supervisors and other health team members regarding patient progress.Documents all required information in the electronic documentation system and maintains patient records.Accurately records treatment minutes for services provided according to requirements.Participates in required meetings including patient care conferences and rehabilitation meetings.Engages in facility and company required trainings, in-services, and conferences.Able to obtain necessary DME or supplies per requirements.Supervises and supports occupational therapy assistants, rehabilitation aides, and students.Adheres to practice acts, codes of conduct, and ethics during professional practice as an Occupational Therapist.
