Soleo Health is seeking a Full-time Therapy Care Pharmacist to support our Acute Therapy Care Management Team in Tempe, AZ. The role involves collecting clinical data, coordinating care plans, and collaborating with prescribers to optimize outcomes in specialty pharmacy services.

The position requires a PharmD or R.Ph with clinical experience, active licensure, and the ability to work on-site. Partial on-call rotation and dedication to quality patient care are essential to success at Soleo

Qualifications

• PharmD or R.Ph with relevant clinical experience.
• Post-graduate residency and/or training is preferred.
• Excellent clinical and customer service skills required.
• Active state licensure and additional specialty training may be required.
• Experience with CPR+ pharmacy system is preferred.

Responsibilities

• Collect and assess clinical data for patients receiving specialty therapies.
• Collaborate with prescribers to implement or modify treatment plans.
• Serve as a clinical resource to patients, prescribers, and team members.
• Coordinate activities with pharmacy team, nursing, and reimbursement staff.
• Document patient interactions clearly in the designated system.
• Dispense specialty medications and supplies.
• Support training of new staff and supervise therapy care specialists.
