Job summary

The LJD Jewish Family And Community Services in Jacksonville, FL seeks a Therapist - Team to assess the clinical needs of parents, caregivers, and children at removal by the Department of Children and Families. You will determine needs for immediate services and make recommendations for ongoing services and parent/child visitation.

Qualified candidates will have a Master’s degree in a related field and Florida licensure eligibility (LMHC, LCSW or LMFT); at least 2 years of experience counseling

Qualifications

  • Education: Masters degree in Social Work, Mental Health Counseling, Marriage and Family Therapy or related field from an accredited college.
  • Experience: 2 years experience counseling children and families preferred.
  • Licensure: Licensed or license-eligible in Florida (LMHC, LCSW or LMFT).

Responsibilities

  • Provides an immediate response (within 48 business hours of the shelter hearing) to assess the child(ren) and caregiver at removal for immediate needs and support.
  • Complete a minimum of 4-6 sessions with the family. Services are provided in the field, usually in the caregiver's home.
  • Meets with the parents to conduct assessments and make recommendations for ongoing services.
  • Most of the work requires being in the field, including home and school visits and removal-home visits.

Job Summary

To assess clinical needs of parents, caregivers, and children at the time of removal by the Department of Children and Families. Determine needs for immediate services and make recommendations for ongoing services and parent/child visitation.

Assess child safety concerns and share information with the assigned Case Manager.

Minimum Qualifications

Education: Masters degree in Social Work , Mental Health Counseling, Marriage and Family Therapy or related field from an accredited college or university.

Experience: 2 years experience counseling children and families preferred.

Knowledge and skills in: Identification of abused and neglected children and adults; child development, individual and family functioning; working with hard to reach or resistant individuals and families; collaborating with other disciplines and services; skills in group dynamics and leading group discussions.

Licensure, Certification, and/or Registration: Licensed or license-eligible in the State of Florida (LMHC, LCSW or LMFT)

Primary Job Functions
  • Provides an immediate response (within 48 (business) hours of the shelter hearing) to the placement home to assess the child(ren) and caregiver at removal for immediate needs and support.
  • Complete a minimum of 4-6 sessions with the family. Services are provided in the field, usually in the caregiver's home.
  • Completes clinical assessments with required information
  • Completes and updates treatment plans as required
  • Documents to reflect all services provided
  • Makes recommendations regarding ongoing services for the children.
  • Meets with the parents to conduct assessments and make recommendations for ongoing services.
  • Most of the work for this position requires being in the field. This will include home visits, school visits, and visits in the removal home with the parents.
  • Coordinates and leads a multi-disciplinary staffing with the assigned case management team within 14 days of the shelter hearing.
  • Maintains a small caseload of ongoing therapy clients. Develops, maintains and completes appropriate clinical records to include treatment plans, treatment plan reviews, addendums, progress notes and bio psycho-socials in a timely manner and in accordance with record management policy.
  • Enters documentation in Florida Safe Families Network (FSFN) and ECr for client records.
  • Participates in a weekly staffing with the QM Coordinator for data collection.
  • Participates in weekly TEAM meetings.
  • Works with all community partners involved to ensure services are delivered in support of the family's case plan
  • Participates in case staffings to discuss the individual/family's progress or response to therapy.
  • Assists in program development and planning as requested or required.
  • Attends trainings, in service sessions, staff meetings and clinical supervision as required.
  • Bills Medicaid and ensures all Medicaid timelines are met.
  • Assists in Quality Improvement issues as requested or required.
  • Develop and maintain therapeutic relationship with families and children
  • Clear a reference check (professional and personal).
  • Clear a local background check from Duval County and the County in which you reside.
  • Clear a drug screen.
  • Provide a copy of your educational attainment on your first day of employment.
  • Clear a Motor Vehicles Records check.
  • Provide proof of your auto insurance and maintain a minimum personal auto bodily injury liability insurance.
