Family Partnerships of Central Florida is seeking a Care Manager Supervisor to oversee the direct supervision and coordination of the casemanagement team serving families in care. You will lead a team of DCMs to identify, assess, and coordinate service delivery for child and family needs, program goals, and contract objectives.

The role requires maintaining Child Protection Certification, overseeing documentation in SACWIS, and ensuring court reports are prepared within timeframes.

Qualifications

Position Summary

Technical/Functional Expectations
  • 1. Completerequiredcertification - Maintain own andstaff's ChildProtection Certification through completionof requiredin-servicetraininghoursandappropriatesupervisionactivities.
  • 2. Safety - Conduct,Develop,Monitorand Managefamilyfunctioningassessments,childsafety assessments and required safetyplans.
  • 3. Permanency -Identifyanddevelopplantosuccessfullyachievepermanencyforchildrenplacedinout-of-homecare.
  • 4. ServiceDelivery-Assesschildandfamilyneeds; develop and monitor serviceplans;coordinate the deliveryof services;ensureallreferralsandlinkages to children/parents/caregiverscompleted.
  • 5. Reports -Monitor/Prepareallrequired courtreports and submitwithintherequiredtimeframes; make recommendations;testifyatcourthearingsregardingreunification,permanent guardianship,terminationof parental rights,or othercase management activities; ensure court orders are completed within required timeframes.
  • 6. Documentation-Develop and maintain, case and program documentation according to contract; ensure documentation occurswithintheSACWISsystem andother relatedsystemswithinrequiredtimeframes. Oversee professional, timelyprogramcasedocumentation and reporting, includingthe trackingand monitoringofclient progressandoutcomes.
  • 7. Engagement-Adherenceto thelocalSOCvalues,goalsandprioritiesfor Brevard'schildren,youth andfamilies; Utilize afamilycentered strength basedapproach tocare;Monitorqualityofcarebyensuringface-to-face home visitswithchildrenarecompleted minimallyevery30days,orwithincreasedfrequency asdeterminedbycase situationsandsupervisorydirection;facilitate,monitor,andassessvisitationbetweenchildren,parents,andsiblings toensuresafetyandobserveinteractions.
  • 8. Support-Providesupport tostaff,children,caregiversandfamilymemberstoassurestableplacement andtimely achievementofpermanency;Coordinatecaseswithteammembers,providers,DCFandattorneysasneeded.
  • 9. CourtAppearanceandPreparation-Ensurestaffhavereviewedcaseinformation priortoeveryhearinginorder to bepreparedtoansweranyquestionsfromtheCourtregardingthestatusofthecase,services, andprogress towards permanency;Dress appropriatelyforCourtappearances andensureallcourthearingsareattended timelybyselfor assignedstaff.

Educational and Experience Requirements

  • 3. ChildWelfareCertification,issuedfrom theFloridaCertification Boardandatleast2years'experienceworking with atriskchildrenandfamiliespreferred.
  • 4. Strongknowledgeaboutdependencycase management andchildwelfaresystemof care.
