Job summary

Senior Health Counseling Services is seeking a compassionate and dedicated full-time therapist to provide mental health services to our skilled nursing and assisted living facilities. Responsibilities include individual counseling, intake assessments, treatment planning, DSM-5 diagnoses, referrals, and meticulous documentation.

Qualifications

• Master's degree and active licensure: LCSW, LPC, LCPC, CMHC, LMFT, or equivalent
• Proficiency with DSM-5 and treatment planning
• Strong documentation and computer skills (Microsoft Word, EHR systems)
• Ability to work independently and manage multiple cases
• Experience with geriatric populations preferred
• Reliable transportation for travel between local facilities

Responsibilities

• Provide individual counseling or psychotherapy to adults and seniors
• Conduct intake assessments and develop individualized treatment plans
