Job summary

Rocky Mountain Care, LLC is seeking a compassionate Part-time therapist to provide mental health services across our skilled nursing and assisted living facilities in Woods Cross and surrounding communities.

Responsibilities include individual counseling, intake assessments, DSM-5 diagnoses, and thorough clinical documentation, with travel between Lindon, Orem, Provo, Payson facilities as needed, while maintaining a flexible schedule.

Qualifications

• Master's degree and active licensure (LCSW/LPC/LCPC/CMHC/LMFT) in Utah.
• Experience with geriatric populations preferred.
• Proficiency with DSM-5 and treatment planning.

• Provide individual counseling or psychotherapy to adults and seniors
• Conduct intake assessments and develop individualized treatment plans

• Offer appropriate resources and referrals as needed
• Maintain accurate and timely clinical documentation
• Manage a personal caseload effectively and independently

Qualifications

• Master's degree and active licensure:LCSW, LPC, LCPC, CMHC, LMFT, or equivalent

• Strong documentation and computer skills (Microsoft Word, EHR systems)
• Ability to work independently and manage multiple cases
