Job summary

Rocky Mountain Care, LLC is seeking a compassionate part-time therapist to provide mental health services in skilled nursing and assisted living facilities across Lindon, Orem, Provo and Payson. The ideal candidate holds a graduate degree and Utah licensure (LCSW/LPC/LCPC/CMHC/LMFT) and can travel between sites.

Responsibilities include individual counseling, intake assessments, DSM‑5‑based diagnoses, treatment planning, and precise clinical documentation.

Qualifications

• Master's degree and active licensure in a relevant clinical field.
• Proficiency with DSM‑5 and treatment planning.
• Strong documentation skills and experience with Word and EHR systems.
• Ability to work independently and manage multiple cases.
• Geriatric population experience preferred, but not required.
• Reliable transportation for travel between local facilities Lindon, Orem, Provo, Payson.

Responsibilities

• Provide individual counseling or psychotherapy to adults and seniors.
• Conduct intake assessments and develop individualized treatment plans.
• Diagnose using DSM‑5 criteria.
• Offer resources and referrals as needed.
• Maintain accurate and timely clinical documentation.
• Manage a personal caseload effectively and independently.
