Job summary

Peak Behavioral Health in Santa Teresa, New Mexico is seeking a Therapist-PRN to provide direct clinical services to patients and families, including psychosocial assessments and individual, group, and family therapy.

Qualifications

  • Master's degree required in Social Work, Counseling or Psychology.
  • At least one year of experience as a therapist with all age groups.

Responsibilities

  • Conduct psychosocial assessments and develop initial discharge plans.
  • Develop and implement treatment plans based on assessments and coordinate additional services as needed.
  • Conduct individual, group or family sessions according to the patient's treatment plan.
  • Apply psychotherapeutic techniques to treat emotional and behavioral disorders.
  • Participate in multidisciplinary treatment planning conferences and present patient information to the team.
  • Maintain a caseload and develop special patient groups or program modules.
  • Prepare progress notes and treatment updates in the medical record, ensuring regulatory compliance.
  • Provide crisis intervention to patients and families in acute distress.
  • Coordinate connections to community resources for post-hospitalization care.

Therapist- PRN | Peak Behavioral Health | Santa Teresa, New MexicoAbout the Job:The Therapist I is a part of the multidisciplinary team responsible for providing direct clinical services to patients and their families with most emphasis on completing Psychosocial Assessments; providing individual, group and family therapy for patients; facilitating psycho-educational training, family meetings, discharge planning; assisting in program development; developing, implementing, and reviewing Treatment Plans; and providing linkage to outside resources.Roles and Responsibilities:Conducts a psychosocial assessment and develops an initial discharge plan.Develops and implements treatment plans for patient based on assessment and coordinates any additional services needed, revising as necessary.Conducts individual, group or family sessions as appropriate for the treatment plan of the patient.Applies psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and behavioral disorders.Participates in multi-disciplinary treatment planning conferences. Presents appropriate patient information to the treatment team. Recommends effective treatment interventions.Maintains a caseload as assigned.Develops and implements special patient groups or program modules.Timely prepares progress notes and treatment updates in the medical record. Maintains all clinical documentation in accordance with regulatory and accrediting standards.Ensures each assigned patient has a structured and clinically appropriate post hospitalization care plan.
