Job summary

Peak Behavioral Health in Santa Teresa, New Mexico is seeking a part-time Therapist I to join a multidisciplinary team. You will provide direct clinical services, conduct psychosocial assessments, and contribute to discharge planning and treatment development.

The role requires a Master's in Social Work, Counseling, or Psychology and at least one year of therapy experience with all age groups; licensed or pursuing licensure is a plus.

Responsibilities

• Conduct psychosocial assessment and develops an initial discharge plan.
• Develops and implements treatment plans for patient based on assessment and coordinates any additional services needed, revising as necessary.
• Conducts individual, group or family sessions as appropriate for the treatment plan of the patient.
• Applies psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and behavioral disorders.
• Maintains caseload and completes progress notes and treatment updates in medical records per regulatory and accrediting standards.
• Provides crisis intervention to patients and families in acute distress.
• Acts as liaison to referral sources and supports continuation of care.

## Therapist- PRNApplylocations: Peak Behavioral Healthtime type: Part timeposted on: Posted 4 Days Agojob requisition id: JR123060Therapist- PRN | Peak Behavioral Health | Santa Teresa, New Mexico**About the Job:**The Therapist I is a part of the multidisciplinary team responsible for providing direct clinical services to patients and their families with most emphasis on completing Psychosocial Assessments; providing individual, group and family therapy for patients; facilitating psycho-educational training, family meetings, discharge planning; assisting in program development; developing, implementing, and reviewing Treatment Plans; and providing linkage to outside resources.**Roles and Responsibilities:**· Conducts a psychosocial assessment and develops an initial discharge plan.· Develops and implements treatment plans for patient based on assessment and coordinates any additional services needed, revising as necessary.· Conducts individual, group or family sessions as appropriate for the treatment plan of the patient.· Applies psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating such diagnosed emotional and behavioral disorders.· Participates in multi-disciplinary treatment planning conferences. Presents appropriate patient information to the treatment team. Recommends effective treatment interventions.· Maintains a caseload as assigned.· Develops and implements special patient groups or program modules.· Timely prepares progress notes and treatment updates in the medical record. Maintains all clinical documentation in accordance with regulatory and accrediting standards.· Ensures each assigned patient has a structured and clinically appropriate post hospitalization care plan.
