Responsibilities

1. Perform comprehensive diagnostic assessments.
2. Conduct scheduled patient visits.
3. Maintain up-to-date treatment plans for each client.
4. Write clear and thorough progress notes.
5. Communicate and follow up regarding client status.
6. Collaborate with case management and psychiatric teams.

Requirements

1. Bachelor's degree in Social Work or related field with one year of therapy experience, or Master's degree in Social Work or Counseling.
2. License such as LSW, LISW, LISW-S, LPC, LPCC, or LPCC-S.
3. Therapy and counseling experience preferred.
4. Experience in home settings preferred.
5. Valid driver's license, insurance, and reliable transportation.
6. Willingness to travel within the service area.
