Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our growing team at River's Bend Retirement Community in Kuttawa, KY. A DPT and an active PT license are required, with NPTE completion to be eligible.

One year of experience is preferred, and the role focuses on evaluating and implementing individualized treatment plans. Applicants will document progress, coordinate with other disciplines, and educate patients and families on home exercise programs and injury prevention.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE must be passed.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
