Job summary

Health Connect America is seeking a Therapist II to provide high-quality mental health services grounded in trauma-informed care. You will conduct assessments, develop and implement individualized treatment plans, and document progress while collaborating with a multidisciplinary team.

Responsibilities include delivering individual, group, and family therapy, monitoring progress, ensuring regulatory compliance, and participating in outreach to promote services and referrals.

Qualifications

• Master’s Degree in Social Work, Counseling, or a related field is required.
• Must hold one of the listed Florida licenses (RMHCI, LPC, LMFT, or LCSW).
• Minimum of one year experience working with children and/or families.

Responsibilities

• Perform comprehensive biopsychosocial assessments to determine needs and interventions.
• Create individualized treatment plans with individuals, families, and the treatment team.
• Deliver therapy (individual, group, and family) and monitor progress.
• Maintain accurate documentation within established timeframes.
• Engage in outreach and collaborate with care teams and external professionals.
