Job summary

Horizon Health and Wellness, Inc. in Arizona seeks a licensed clinician to provide therapy to children, families and adults, including assessments, evaluation, diagnosis, treatment and case management.

The role requires a Master’s degree in counseling, psychology, social work or related field, AZ licensure (LMSW/LAC/LASAC/LAMFT or equivalent), valid driver’s license and the ability to work evenings; strong communication and cultural sensitivity are essential.

Qualifications

• Master’s degree in counseling, psychology, social work, or related field.
• Arizona licensure (AzBBHE LMSW/LAC/LASAC/LAMFT or Licensed Independent Substance Abuse Counselor) required.
• Experience with best-practice therapeutic approaches and strong communication skills.

Responsibilities

• Provide therapy to children, families and adults including assessments, evaluation, diagnosis, treatment and case management.
• Maintain client records in current status and ensure documentation is complete and accurate.
• Demonstrate ability to work within a multi-disciplinary team and handle sensitive information.
