Job summary

Horizon Health and Wellness, Inc. in Arizona seeks a licensed clinician to provide therapy to children, families, and adults, including assessments, diagnosis, treatment, and case management.

You will maintain client records, collaborate with a multidisciplinary team, and meet licensure and safety requirements. Evening availability, valid AZ license, and bilingual skills (English/Spanish) are a plus.

Qualifications

• Master’s degree in counseling or related field.
• AzBBHE licensure (LMSW/LAC/LASAC/LAMFT or Licensed Independent Substance Abuse Counselor) required.
• Experience with best-practice therapeutic approaches.
• Must be 21+ years old.
• Valid driver’s license and reliable transportation.
• Family therapy knowledge/experience.
• DSM-IV knowledge.
• Willingness to work evenings.
• Ability to develop treatment plans.
• Biopsychosocial assessment skills.
• Ability to operate electronic clinical records.

Responsibilities

• Provide therapy to children, families, and adults.
• Conduct assessments, evaluation, diagnosis, and treatment.
• Maintain client records and case management duties.
