Job summary

Health Connect America, Inc. is seeking a licensed therapist to provide high-quality mental health services.

Responsibilities include conducting biopsychosocial assessments, creating and implementing individualized treatment plans, and delivering therapy to individuals, families, and groups. The role involves documentation, crisis intervention, and collaboration with a multidisciplinary team to support client goals.

Qualifications

• Master’s Degree required in SW, Counseling or related field.
• Must be eligible for licensure as LPC, LMFT, or LCSW in relevant state.
• Experience in delivering mental health therapy across populations.

Responsibilities

• Perform biopsychosocial assessments to determine needs and interventions.
• Develop and implement individualized treatment plans with clients and families.
• Maintain accurate documentation of treatment and progress.
• Deliver individual, group, and family therapy sessions.
• Provide crisis intervention and safety planning as needed.
• Engage in outreach to promote services and referrals.
• Collaborate with multidisciplinary team for holistic care.
• Ensure regulatory compliance and timely progress notes.
