Qualifications

Responsibilities

• Develop comprehensive clinical assessments on assigned individuals and families.
• Serve as the ongoing liaison with community agencies and referral sources.
• Complete discharge summary and assist with discharge planning on each assigned case.
• Provide aftercare services for assigned cases post-discharge.
• Meet at least once a week with treatment team members to coordinate treatment planning.
