CommuniCare Health Partners in Indianapolis seeks Licensed Social Workers (LSW/LCSW) or Licensed Mental Health Counselors (LMHC) to provide psychotherapy and care management in long-term care settings. You will collaborate with psychiatrists and primary care providers to address chronic severe mental illness and neurocognitive disorders.

Applicants must hold an Indiana license and a master’s degree in social work or counseling, with EMR proficiency and strong teamwork skills.

Qualifications

• Current LSW, LCSW, or LMHC licensure in Indiana.
• Master’s Degree in Social Work or Master’s Degree in Counseling from an accredited institution.

Responsabilités

• Deliver individual and group psychotherapy sessions tailored to residents.
• Facilitate collaborative care management with psychiatrists and primary care providers.
• Address mental health needs of residents with chronic severe mental illness and neurocognitive disorders.
• Develop and implement evidence-based therapeutic interventions.
• Maintain detailed clinical documentation in an EMR system.
• Conduct initial behavioral health evaluations as needed.
