Position Overview:

We are seeking Licensed Social Workers (LSW) and Licensed Independent Social Workers (LISW) to join as Therapist/Behavioral Health Care Managers. In this role, you will provide psychotherapy, group therapy, and collaborative care management to residents in need.

Key Responsibilities:

• Deliver individual and group psychotherapy sessions tailored to residents' needs.
• Coordinate care with psychiatrists, primary care providers, and team members.
• Address mental health needs of residents with chronic severe mental illness, substance use disorders, depression, and neurocognitive disorders.
• Develop and implement evidence-based therapeutic interventions.
• Maintain detailed clinical documentation in an electronic medical records (EMR) system.

Qualifications:

• Current LSW or LISW licensure in Indiana.
• Master’s Degree in Social Work from an accredited institution.
• Experience with diverse patient populations, including those with severe mental illness and neurocognitive disorders.
• Proficiency in electronic medical records (EMR) systems.
• Familiarity with evidence-based therapeutic modalities (preferred).
• Strong interpersonal and communication skills.
• Ability to work collaboratively within a multidisciplinary team.
