Job Description

  • Purpose: Responsible for providing individual, group and family therapy for both mental health and substance abuse treatment, the therapist participates in interdisciplinary treatment planning by developing an annual comprehensive psychosocial assessment and crisis intervention consultation. Provides individual and family counseling, makes referrals to external services, makes referrals for internal psychiatric services, and assists clients in identifying resources to meet their needs. Some remote/hybrid work may be assigned.

PART I: ESSENTIAL POSITION FUNCTION AND DUTIES

  • Provides individual, group and family therapy.
  • Participates in interdisciplinary treatment planning and warm handoffs from other providers in practice.
  • Develops an annual comprehensive psychosocial assessment for clients.
  • Provides crisis intervention consultation.
  • Participates in the peer-review process for the therapist team.
  • Provides pre-screening for depression, anxiety and other psychological disorders.
  • Conducts individual and family counseling sessions with clients.
  • Evaluate and diagnose psychological disorders.
  • Conducts psycho-social assessments
  • Provides case management
  • Contacts social service agencies on behalf of clients to obtain services
  • Makes referrals to other agencies and psychiatrist as appropriate.
  • Manages patient appointments
  • Schedule patients with psychiatrist, completes pre-visit evaluation and preps patient chart for visit with psychiatrist
  • Prepares documentation for patient files of visit and other notes
  • Completes electronic charting in EMR.
  • Attends all assigned meetings and huddles.
  • Completes superbills, electronic billing within required time frame.
  • Completes all documentation in patient chart within required time frame.
  • Performs other duties as assigned.

  • Required to work closely with patients and associates.
  • Interacts with other departments on problem issues.
  • Accepts feedback from patients, visitors, clinic employees, physicians and general public.

  • Continuously looks for and suggests ways to improve.
  • Effectively completes assignment to achieve the greatest benefits at acceptable cost.
  • Implements improvements as appropriate.
  • Demonstrates interest in own growth and development by:
  • Periodically evaluating own performance.

  • Attends and participates in in-services and continuing education programs
  • Attends departmental meetings.

  • Understands that all confidentiality and privacy considerations are respected and fostered at work and off duty.
  • Self –Management:
  • Presents a positive image of Western Wayne Family Health Centers at all times.
  • Carries out assignments with little need for direction.
  • Timeliness.
  • Maintains confidentiality.
  • Provides proper notification of absence and tardiness.
  • Works weekends and shifts when necessary.

PART II: EDUCATION, EXPERIENCE

  • Any fully licensed mental health professional, (i.e., LMSW, LPC) to provide mental health and/or behavioral health services in the State of Michigan with a minimum of two years experience.
  • Limited Licensed Mental Health Professional who are in the process of obtaining full licensure within 6 – 12 months. *Documentation is needed upon interview.

PART III: KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge of health care administration systems.
  • Knowledge of governmental regulations and reimbursement requirements.
  • Knowledge of computer, systems, and applications.
  • Skilled in planning, organizing and supervising patient care.
  • Skilled in exercising initiative, judgment, problem-solving, decision making.
  • Skilled in developing and maintaining effective relationships with medical and administrative staff, patients and the general public
  • Proficiency in Microsoft Excel and knowledge of other Microsoft Office programs
  • Skilled with working in a complex work environment
  • Ability to maintain confidentiality at all times and maintain organizationally appropriate relationships
  • Knowledge of organization policies and procedures.
