Job summary

RHA Health Services, LLC is seeking a Therapeutic In-Home Coach for Wilmington, NC, to support START program participants with developmental disabilities and mental health challenges. The role emphasizes community-based interventions, crisis management, and collaboration with families and healthcare providers.

The position requires a Bachelor’s degree in psychology, social work or related field with relevant experience.

Qualifications

• Bachelor’s degree in psychology, Social Work or other Human Service-related field required with 2 years’ experience with population served preferred

Responsibilities

• Coaches work with the home or other community settings to create and implement person specific and crisis therapeutic interventions and communicate these interventions with primary caregivers.
• Coaches contribute to the overall crisis stabilization and prevention of the individual and family by providing hands-on assistance and education to families.
• Coach participates as part of comprehensive START services that collaborate with individual’s primary provider and clinical home, community crisis services, family and others.
• Offer consultation and hands-on education with entire family, support system to create a safe, nurturing, and meaningful environment for families and individuals with developmental disabilities.
• Assists individuals with implementation of daily activities, behavioral health needs, and health, wellness, and leisure activities.
• Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation support in accordance with crisis guidelines and person’s plan.
• Maintains timely documentation of session and visits notes and other data.
• Must be self-motivated, efficient with time
• Coaches work with families and individuals to work on identified needs/skills to assist with strategies to improve coping mechanisms
• Coaches help to develop strategies and tools to assist others with improving overall well-being.
