Job summary

Clarvida is seeking a Care Coordinator to provide approved therapeutic interventions, case coordination, and resource linkage to clients and families to support identified changes. The role requires collaboration with treatment teams, documentation, and active participation in supervision.

The position emphasizes culturally sensitive care, adherence to policy, and the ability to travel to support community-based services.

Qualifications

• Bachelor’s degree in a Human Service discipline from an accredited four-year college or university.
• 1–2 years of relevant experience in the Mental Health field and Case Management.
• Sensitive to other cultures and socioeconomic levels.
• Has thorough knowledge of child development and behavior modification.

Responsibilities

• Assessing clients on intake as needed, conducting community client visitation, transporting clients.
• Providing Clarvida approved therapeutic services to clients and families including coping skills, behavior modification, parenting skills, and crisis intervention.
• Working with treatment teams to create, personalize, and update Treatment Plans and ensure compliance with policies and regulations.
• Serving as a liaison and client advocate with other professionals and agencies.
• Attending supervision and treatment team meetings and presenting on assigned clients as required.
• Supporting team members to ensure the best outcomes for all Clarvida clients and families.
• Completing clinical documentation within established time frames and rectifying deficits promptly.
• Contributing to Quality Improvement through data collection and audits.
