Clarvida is seeking a Care Coordinator to provide approved therapeutic interventions, case coordination, and resource linkage to clients and families, including collaboration with the treatment team and external agencies.

The role involves conducting intake assessments, updating treatment plans, liaising with professionals, attending supervision meetings, and ensuring timely clinical documentation to support quality care outcomes.

Formación

• Bachelor’s degree in a Human Service discipline from an accredited four-year college or university.
• 1-2 years of relevant experience in the Mental Health field and Case Management.
• Sensitive to other cultures and socioeconomic levels.
• Has thorough knowledge of child development and behavior modification.
• Experience working with children/adolescents and adults in a therapeutic, community-based treatment environment.

Responsabilidades

• Assessing clients on intake as needed and providing Clarvida approved therapeutic services.
• Working with treatment teams to create and update Treatment Plans.
• Serving as liaison and client advocate with other professionals.
• Attending supervision and treatment team meetings and presenting on clients.
• Supporting team members to achieve the best outcomes for clients and families.
• Completing clinical documentation within established timeframes and rectifying deficits.
• Contributing to Quality Improvement by data collection and analysis.
