Job summary

Sage Eldercare Solutions in Seattle area seeks a part-time Quality of Life Engagement Specialist to plan and deliver Hummingbird Services in client homes throughout North Seattle and King County. This role offers 15-20 hours per week with hourly pay of $39–$41.

Direct services constitute about 85% of paid time, with creative, person-centered activities that enhance quality of life. You will conduct intake calls, home assessments, write Life Plans, and support Home Care clients with activities,

Qualifications

• Mental Health Professional or comparable training in Therapeutic Arts.
• Minimum 5 years of experience with older adults.
• Empathetic, compassionate with a creative approach to activity plans.

Responsibilities

• Direct Service: planning and delivery of Quality of Life services.
• In-home assessments and development of Quality of Life Plans.
• Documentation and communication with elder care professionals.
• Community relationship-building to expand referrals and collaborations.
