Job summary

Sage Eldercare Solutions is seeking a part-time Quality of Life Engagement Specialist to perform in-home consultations across the East Bay, including Oakland and surrounding areas. The role focuses on developing individualized engagement plans and delivering therapeutic sessions to improve clients’ overall well-being.

Responsibilities include comprehensive assessments, planning, documentation, and collaboration with families, fiduciaries, and care teams.

Qualifications

• Strong relationship-building and therapeutic engagement abilities.
• Excellent interpersonal, written, and verbal communication skills.
• Ability to work independently while collaborating within interdisciplinary teams.

Responsibilities

• Conduct comprehensive quality of life consultations, assessments, and intake visits.
• Develop individualized Quality of Life Engagement Plans based on client needs.
• Deliver engagement sessions using evidence-informed approaches.
• Document plans and progress, maintaining organized records and schedules.
• Communicate changes in client status with families and care teams.
• Build relationships with referral partners and participate in outreach.
