Job summary

Sage Eldercare Solutions seeks a part-time Quality of Life Engagement Specialist to deliver in-home or community-based services throughout the South Seattle/KING County region. The role focuses on planning and delivering quality of life activities, with 85% direct service time and close collaboration with the Hummingbird Project team.

Ideal candidates are creative, self-driven professionals with experience in aging, arts, or therapeutic activities, and a passion for improving clients’ overall

Qualifications

• Mental Health Professional or comparable training and experience in Therapeutic Arts; OR a bachelor’s degree and 10 years of experience working with older adults using art and recreation.
• Minimum of 5 years of experience working with older adults.
• Empathetic and compassionate with a creative approach to developing individualized activity plans.
• Proficient in holistic quality of life, life enrichment, and engagement practices.
• Exceptional interpersonal skills with the ability to engage and comfort a diverse range of individuals.
• Excellent written communication skills and time management abilities.
• Experienced with G-Suite and video communication platforms like Zoom, FaceTime, and Google Hangouts.
• May transport clients as part of job responsibilities. Must maintain a valid driver’s license and meet the organization’s auto insurance requirements.

Responsibilities

• Direct service: planning and delivery of Creative, High Quality Hummingbird Services.
• Conducts initial phone calls with prospective clients and community members; assess quality of life interests and needs.
• Conducts in-home assessments and develops written Quality of Life Plans focusing on creativity and joy.
• Implements all aspects of Quality of Life Plans and overcomes barriers.
• Develops new Quality of Life activities reflecting best practices.
• Updates QoL Plans to meet changing client needs and introduces new ideas.
• Maintains written engagement plans and communication with elder care professionals.
• Builds relationships with the community to generate referrals and collaborations.
• Shares knowledge with the team and contributes to monthly meetings.
