Kona Medical Consulting is seeking a Home Health Field Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients’ homes across Texas. The role supports an RN Case Manager and requires dependable, clinically confident professionals who thrive in autonomous field settings.

Qualifications

• Current Texas LVN license.
• 1+ year nursing experience (Home Health preferred).
• Strong assessment, communication, and clinical skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Familiarity with EMR/mobile charting.

Responsibilities

• Provide skilled nursing care under supervision of RN Case Manager.
• Perform wound care, medication administration, vitals, and disease process teaching.
• Assist with care plan implementation and updates.
• Conduct patient assessments and report changes in condition promptly.
• Collaborate with physicians, RNs, and other team members to ensure quality care.
• Maintain timely, accurate documentation in the EMR.
• Travel to patient homes within an assigned service area.
