Kona Medical Consulting and The Home Health Team is looking for a Home Health Field LVN to provide skilled nursing care in patients' homes across Texas. The role supports independence at home with a strong clinical team backing you up.

The ideal candidate has a Texas LVN license and 1+ year of nursing experience, with proficiency in wound care, med administration, vitals, and EMR documentation. Travel within assigned service areas is required.

Qualifications

• Current Texas LVN license
• 1+ year of nursing experience (Home Health preferred but not required)
• Strong assessment, communication, and clinical skills
• Reliable transportation, valid driver's license, and auto insurance
• Familiarity with EMR/mobile charting
• Compassionate, dependable, and patient-focused

Responsibilities

• Provide skilled nursing care under the supervision of an RN Case Manager
• Perform wound care, medication administration, vitals, and disease process teaching
• Assist with care plan implementation and updates
• Conduct patient assessments and report changes in condition promptly
• Collaborate with physicians, RNs, and other team members to ensure quality care
• Maintain timely, accurate documentation in the EMR
• Travel to patient homes within an assigned service area
