Kona Medical Consulting is seeking a Home Health Field LVN to deliver skilled nursing care in patients’ homes across Texas. You’ll work independently, supported by a clinical team, and travel within an assigned service area.

The ideal candidate holds a Texas LVN license with 1+ year of nursing experience. Strong assessment, communication, and clinical skills are essential, along with reliable transportation and EMR familiarity. Competitive pay and growth opportunities are available.

Formación

• Current Texas LVN license
• 1+ year nursing experience in home health preferred but not required
• Strong assessment, communication, and clinical skills
• Reliable transportation, valid driver’s license, and auto insurance

Responsabilidades

• Provide skilled nursing care under the supervision of an RN Case Manager
• Perform wound care, medication administration, vitals, and disease process teaching
• Assist with care plan implementation and updates
• Conduct patient assessments and report changes in condition promptly
• Collaborate with physicians, RNs, and other team members to ensure quality care
• Maintain timely and accurate documentation in the EMR
• Travel to patient homes within an assigned service area
