Work Schedule: Friday-Monday or Saturday-Tuesday, 8:00 am - 7:00 pm PST, must be willing to work Pacific Time Zone

What You Need To Know

The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of referrals based on predetermined criteria set by the Agency. This role may cover one or more states within AccentCare’s operations and may change according to company needs. The coordinator establishes and maintains communication with clients, family members, and referral sources, partnering with agency staff, referral sources, and sales to ensure patient satisfaction and outcomes. They facilitate obtaining all necessary information and documentation to complete referrals through follow-up with the agency, referral source, and client.

Qualifications

Be the Best Territory Patient Admissions Coordinator Clinical You Can Be

• Licensed RN, LVN, or PT in practicing state
• 3-7 years of experience in facility/physician relationships, with a deep understanding of facility discharge processes (preferred)
• Minimum of one year experience in home care, hospice, or personal care services (preferred)
• Prior use of Electronic Medical Records (EMR) (preferred)
• Experience dealing with various healthcare payors such as Medicare, Medicaid, and commercial payers (preferred)
