Homestead Healthcare seeks an experienced Community Liaison to drive referral generation and business growth for our home care services. This role requires ownership of the territory, established relationships, and accountability for referral metrics impacting profitability.

The position emphasizes independent work, weekly partner engagement, and meeting quarterly referral targets while coordinating with intake and care teams to onboard referred clients.

Qualifications

• 3+ years of experience in community outreach, marketing, or liaison role in healthcare.
• Proven track record of meeting or exceeding referral quotas.
• Strong network of referral sources in the local counties.
• Excellent communication, networking, and presentation skills.
• Self-motivated, proactive, and results-focused.
• Ability to work independently and manage your schedule.

Responsibilities

• Develop and execute a referral growth strategy targeting hospitals, SNFs, assisted living, hospice, physician offices, and other partners.
• Leverage relationships to generate qualified referrals for home care services.
• Maintain a weekly presence at referral partners to strengthen engagement and drive flow.
• Meet or exceed monthly and quarterly referral targets defined by leadership.
• Track and report referral activity, relationship status, and pipeline using CRM or tracking tools.
• Provide feedback on market trends and competitor activity to inform strategic decisions.
• Represent the company at community events, health fairs, and networking meetings.
• Collaborate with intake and care coordination to ensure timely onboarding of referred clients.
• Flexibility to assist in capacities as needed.
