Qualifications

• 2 years of experience in the human services field or a bachelor’s degree in Social Work or in a related field
• Possess a valid California driver's license and have access to a properly registered vehicle

Responsibilities

• Provide intensive case management to clients living in a permanent supportive housing program who also have a chronic health condition
• Conduct intake and enrollment with eligible clients
• Assist clients with accessing temporary housing until permanent housing is secured
• Support clients with the lease‑up process
• Conduct comprehensive bio‑psycho‑social assessments
• Develop individualized collaborative service plans
• Assist clients with their physical and mental health needs by providing support and linkage to appropriate services
• Transport clients as needed to essential appointments
