Job summary

Grand Street Settlement is seeking a Home Visitor for a temporary position at our Sunset Park Child and Family Center in Brooklyn. The role focuses on providing culturally competent child development and parent support services, with emphasis on helping families access health documents and community resources.

The Home Visitor will maintain data records, support prenatal and postnatal care, and collaborate with a multi-disciplinary team to promote family well-being and school readiness.

Qualifications

• High School Diploma with CDA credential or comparable credential.
• 60-90 college credits in Social Work, Human Development or Early Childhood Education preferred.
• Experience with young children and understanding of adult learning principles.
• Experience with community resources and strong written/spoken communication.

Responsibilities

• Maintain a caseload of around 12 participants and conduct weekly home visits.
• Support prenatal and postpartum care and provide prenatal education.
• Collaborate with supervisor to plan developmentally appropriate parent/child activities twice per month.
• Assist families in identifying goals and completing the Family Partnership Agreement.
• Enter and maintain data in COPA/TSG and participate in program assessments.
